Code 15 - 1994 Honda Accord EX 2.2L V4
Cranked the car a couple of weeks ago, started fine, but when the RPM's evened out the car died. I let it sit for a few minutes and tried again, the same thing happened. Checked my distributor and rotor both looked ok, rotor is a little old but looked ok. Checked all my connections everything was solid. Checked my code and showed code 15. Looked it up it, says insufficient spark. Check forum after forum, all tell me it's the ICM (ignition control module). I change the ICM and everything was back to normal... Until today. Same thing happened and the same code was given. Any help please?

Actually IIRC, code 15 is something like "ignition output signal". A 1994 ECM cannot actually measure anything about the spark itself.
The ECM sends a signal to the ignitor telling it to fire the coil. If that signal isn't working, it might be a bad ignitor, or it might be an intermittent problem with the wiring from the ECM to the distributor. Have you checked the wiring before replacing the ignitor? Maybe a loose/dirty/corroded pin in the wiring harness somewhere? How about the spade connectors at the ignitor (inside the distributor)? Those kind of things...

It will crank, but as the RPM's level off the car dies. Doesn't matter if I give it gas or not. I found one of my distributor cap bolts is stripped out, and not allowing the cap to close well. Might this be affecting it too?
