Could be:
1) Bad fuel pressure regulator. A bad one can leak flood the cylinders causing the engine to misfire.
2) Bad coolant temperature sensor. Could send a wrong reading to the ECM telling it the engine is warm when it's actually cold. This would affect the air/fuel ratio and cause performance problems.

Or are we are overlooking the normal things. Can almost rule out the ign switch, I think 98 was about the first year of the recall. Never dies just boggs down or can't hold speed.
Then next on the list would be the main relay (MFR ?) acticing up.
But of course the oil inside the dist can also lead to strange things.

Chad did you figure it out?
I have a 98 Honda Accord EX V6 with 216,000 miles that is doing the exact same thing. We have taken it to two reputable mechanics and they have not been able to figure out what was wrong. We did have a check engine light on and the code came up for the EGR valve as well, which I had previously replaced with a new one. We had some speculate it was o2 censors as well. We are in the same boat and don't want to pay a lot of money for a fix that may or may not work. Thanks so much for posting in the first place and hopefully it will be a easy fix.

As always check for codes first, then if there are no codes.
When engine is warm it goes into "closed loop". So egr, O2 and a few other things "go into action".
Now some will say if there was an issue with these there should be a code set, well guess what, not always.
Lazy/slow O2 can give false/slow readings to the ECU. Some can be test with a volt meter
EGR ports/valve cleaning should almost be considered reg maint.
800rmk- your gen has a very solid auto trans (keep the fluid changed and it will outlast the body, IMO). If you have no codes I would suggest cleaning inspecting the egr valve and ports. Next I would check the O2.

poorman, I did have a blinking CEL last year and verified the egr operation. I also cleaned the egr ports that were 3 out of 4 plugged solid. No CEL since, but just the surging/hesitation as previously discussed. Have also verified the shift/lockup solenoids operation, since it "felt" like the transmission is up/downshifting eratically when everything gets up to operating temperature. But, I do have a genuine honda shop manual and will see how to check the 02 sensor. Thanks
