Was there water/coolant in the oil when you first tore the engine down? If so it is something that was missed on tear down. I can only picture a crack in a cylinder wall or something similar causing this other than something out of place such as a wrong head gasket.

Sorry for my delayed response. I am way, way backed up on other auto projects and this Accord is gonna have to take a back seat for a tad.
Yes, the oil was milkshaked when I got it. It was hard to tell if the headgasket was blown because on the tear down, so much of the black sealing compound on the gasket was gone or stuck to the mating surfaces.
It was the right gasket, and the replacements were the same, installed correctly. Hopefully the problem will present itself on this next teardown, fingers crossed !
